Proof of relationship with the head of the family will establish the applicant’s identity.
2. In the introducer option, introducers are available at the enrollment centres, and the Registrar notifies the Introducer. Introducers are individuals like registrar’s employees, elected local body members, individuals of local administrative offices, Anganwadi workers or Asha workers. For more details, visit an Enrollment centre for the Introducer option.
Fill up the enrollment form either downloaded or at an enrollment centre. Provide documents about the relationship proof with the child. For a child below five years of age, the child’s photo and any parent’s Aadhaar copy are required.
